Important Forms and Checklist

Now that you have been accepted, we need important information from you to begin your journey at SUA. You should have already received a packet with the Welcome Brochure and following forms. In case you did not receive the packet, or if you need another copy of the forms, they are readily accessible in PDF format below. General Information and Schedule 

Forms due by July 2, 2018:

  • Student Affairs Checklist (PDF) - You can use this list to check off the forms we need from Student Affairs
  • Student health history form (PDF) - All students must complete this form and meet the immunizations requirement in order to register for classes
  • Student health insurance election/waiver Form (PDF) - All incoming students MUST have health insurance. The annual premium for SUA's Student Health Plan is $1836 (for each session, $918) and is underwritten by Anthem Blue Cross. Please see the brochure below, which outlines the specific benefits. All international students are required to purchase SUA health insurance. Domestic students may waive the health insurance fee by submitting this waiver and providing a current copy of the student's insurance card. Your insurance must be able to provide coverage in California.  
  • Soka Student Health Insurance Brochure 18-19 (PDF) - Our insurance is a basic indemnity plan which provides an illness/injury benefit that includes a zero deductible, a co-payment of $10 for generic prescription drugs and unlimited maximum benefit for the year. Please be aware that Soka only offers medical insurance, and DOES NOT include vision or dental.  
  • Orientation Activities Liability and Photo Release Form (PDF) - This is a waiver for Orientation activities that will be taken place off campus. 
  • Residence Hall Questionnaire - Due to technical difficulties that some students are facing, Res Life is extending the deadline for First Year Residence Hall Questionnaire to July 6, 2018.  It will be used to make roommate assignments. For more information on housing, please refer to the Housing page. Your housing placement details will be available to view online after 5:00pm PST on July 16, 2018.  Please note that all students are required to check in at the residence halls between August 5-6, 2018.
